The evaluation will try to reflect the particular issues arising at the various stages of programme implementation. For example, in the early stages we will focus on the way activities are being developed as well as identifying any early successes. As the programme matures there can be a gradual transition to assessing the achievement of objectives. In year 3, there is a
requirement that we will formally report on progress towards meeting national targets.

Sure Start Mellow Valley is a 6 th wave Sure Start programme, currently in its second
year. The Children’s Society is the lead partner and accountable body. The
programme was approved in April 2003. Since then the team has grown – for a full
list of current Sure Start staff, please Appendix A. Sure Start Mellow Valley has
established links with many other local initiatives that aim to improve the lives of
families and children aged 04
years and promotes an inclusive approach to the
development of working with local agencies, organisations and funders.
Sure Start Mellow Valley is characterised by high levels of parental involvement.
Indeed, parents in Little London & Woodhouse and in Meanwood were closely
involved in forums/groups lobbying for a Sure Start programme. The Parents Forum
groups are always well attended and parents are also represented on the management
board.
It is a requirement for all local Sure Start programmes to undergo a process of
evaluation to determine whether the programme is meeting the needs and expressed
wishes of the population it serves. The Centre for Health Promotion Research at
Leeds Metropolitan University has been commissioned to evaluate Sure Start Mellow
Valley. An initial Baseline User Satisfaction Survey formed the first stage of the
evaluation. It was conducted in Summer 2004. This report details the methods used
and findings from the survey.

Sure Start is a government programme which aims to make a difference to
children 0-4 years and their families through supporting parents, increasing
childcare availability and improving the health and social development of young
children. Local Sure Start programmes seek to involve parents and other
partners in their work. Sure Start principles are listed below. Sure Starts will be

CHILL (Community Health Information and Links, Leeds) is a new project to train and support volunteers that is being developed and delivered by Women’s Health Matters (WHM). The broad aims of the project are to enhance the knowledge, skills and confidence of individual volunteers, and to increase organisational and community capacity through the role of the volunteers in supporting women’s health groups and other community activities.
The project works with women from a range of often marginalised backgrounds and many are disabled, have experience of mental and physical ill-health, discrimination and domestic violence. According to one staff member, it aims to enable women to “cross… barriers in terms of personal experience”. This is achieved through the delivery of a programme of activities which will both facilitate increased confidence for individual participants, provide opportunities for them to learn about issues affecting women in general, and also the range of issues faced by women the organisation works with. This includes multiple forms of discrimination and barriers to accessing services. Another dimension of the project is to enable volunteers to develop skills necessary in setting up, facilitating and managing groups. Training is delivered through a combination of means: by WHM staff and, where necessary, by people with

Public Health Nursing is an essential resource for all health visiting students, school nursing students, and occupational health nursing students, that reflects the current key changes in community public health nursing. It is a key textbook for specialist practitioner programmes, and those new to the public health arena.

Written by relevant experts in the field, this practical textbook uniquely explores the three main specialties of Public Health Nursing: Health Visiting, School Nursing and Occupational Health Nursing. A particular strength of the book is the way it shows the diversity of each discipline and how they each address Public Health in vastly different ways according to the needs of their relevant population.

This will be essential reading for all students on the Specialist Community Public Health Nursing (SCPHN) programmes offered across the UK.

The DAMASK trial protocol: a pragmatic randomised trial to evaluate whether GPs should have direct access to MRI for patients

Background: Though new technologies like Magnetic Resonance Imaging (MRI) may be accurate, they often diffuse into
practice before thorough assessment of their value in diagnosis and management, and of their effects on patient outcome
and costs. MRI of the knee is a common investigation despite concern that it is not always appropriate. There is wide
variation in general practitioners (GPs) access to, and use of MRI, and in the associated costs. The objective of this study
was to resolve uncertainty whether GPs should refer patients with suspected internal derangement of the knee for MRI
or to an orthopaedic specialist in secondary care.
Methods/Design: The design consisted of a pragmatic multi-centre randomised trial with two parallel groups and
concomitant economic evaluation. Patients presenting in general practice with suspected internal derangement of the
knee and for whom their GP was considering referral to an orthopaedic specialist in secondary care were eligible for
inclusion. Within practices, GPs or practice nurses randomised eligible and consenting participants to the local radiology
department for an MRI examination, or for consultation with an orthopaedic specialist. To ensure that the waiting time from GP consultation to orthopaedic appointment was similar for both trial arms, GPs made a provisional referral to
orthopaedics when requesting the MRI examination. Thus we evaluated the more appropriate sequence of events
independent of variations in waiting times. Follow up of participants was by postal questionnaires at six, twelve and 24
months after randomisation. This was to ensure that the evaluation covered all events up to and including arthroscopy.
Discussion: The DAMASK trial should make a major contribution to the development of evidence-based partnerships
between primary and secondary care professionals and inform the debate when MRI should enter the diagnostic pathway.
